Telemedicine for cancer patients with acute infection symptoms at a local hospital

Introduction
We applied a telemedicine to enable diagnosis and treatment for acute infection at a local hospital with no infection specialist.
Methods
18 patients with acute infection signs (fever < 2 days, white blood cell < 3,500 cells/mcL) were studied. 10 patients were evaluated by study physicians and an infection specialist via telemedicine 3 times a week (TM). 8 patients by usual physicians only (UL). One empiric antibiotics was prescribed to every patient at the first 3 days.
Results
At TM,4 were changed the initial diagnosis and added more antibiotics. The average fever subsided on day 5 (7 for UL). The average hospital stay was 10 days (14 for UL ).
Conclusions
A infection specialists experience and knowledge could improve patient’s outcomes at local hospitals which lack of infection specialist. Video visit could reduce in-person visits. Mortality rate did not decrease, but the outcome for survivals increased.
