Experiences of Counseling Participating Hospitals to Promote the Bundle Care in Northern Taiwan

Background: Hospital-associated infections (HAIs) is an important indicator of patient safety at hospitals. Evidence-based interventions have been shown to reduce the incidence of ventilator-associated pneumonia (VAP) and catheter-associated urinary tract infections (CAUTI).
Methods: To reduce invasive catheter-associated infections, we perform 3-year project counseling 4 hospitals in northern Taiwan, including 2 regional hospitals and 2 district hospitals. The project was divided into the pre-intervention phase (from January 1, 2014, to June 30, 2015) and post-intervention phase (from July 1, 2015, to September 30, 2016). Prospective surveillance was performed on all patients admitted to these hospitals with a consensus definition.
Results: During the project period, there were 117,046 patient-days, and 87,692 ventilations-days with a total of 102 episodes of VAPs in the participating hospital identified. VAP incidence density remarkably decreased from 1.37 to 0.98 per 1000 patient-days (P=0.05). In addition, 477 episodes of CAUTIs were assessed in 542,609 patient-days over 174,851 catheter-days. Comparing the incidence density of CAUTI before to after the project decreased from 3.35 to 2.25 per 1000 patient-days (P<0.001).
Conclusion: Our interventions in 4 northern Taiwan hospitals had revealed successful decreasing HAIs. All hospitals should consider promoting the bundle care to improve medical care quality.
