A Family Cluster of Parvovirus B19 Infection Mimicking Measles in Taiwan

Background: In April 2018, there was an outbreak of measles in Taiwan transmitted on board a commercial aircraft by a Taiwanese passenger en route to Japan. Majority of patients had modified presentations as they had been vaccinated in childhood.
Materials: We describe a cluster of parvovirus B19 in a family with clinical illnesses mimicking measles infection, and outline the diagnostic pitfall of reliance on IgM for diagnosis of viral exanthemata.
Results: Four members of a single family suffered from fever and/or rash. Two had cough, one had exanthema, and one had conjunctivitis and arthralgia, who also had detectable rubella and measles IgM in his serum. All 3 patients with exanthema had parvovirus B19 viremia detected by PCR.
Conclusion: Differential diagnosis of febrile exanthema remains challenging among vaccinated populations with modified presentations. Without seroconversion, diagnosis should be reinforced by other tests including nucleic acid detection or antibody avidity.
