The comorbidity risk of elderly patients with Pneumococcal pneumonia

Introduction:Elderly patients are usually a major group for getting Pneumococcal pneumonia. Large-scale population-based studies of elderly patients with different co-morbidity diseases for having Pneumococcal pneumonia in Taiwan are rare.
Method:The longitudinal national health insurance research database was applied in this study. From 1997 to 2009, 702 elderly patients (age>=65) with Pneumococcal pneumonia and 2106 age-and-gender matched controls were enrolled. The logistic regression analysis was used to perform the risk of comorbidities of elderly patients with Pneumococcal pneumonia.
Results:Our results showed the distribution of elderly patients (age>=65) with Pneumococcal pneumonia decreased from10.9% in 1999 to 6.3% in 2009. Compared with patients without streptococcus pneumonia, the adjusted odds ratio show positive risk probability of co-morbidities for elderly patients with Pneumococcal pneumonia, including heart failure: 1.61 (95% C.I.: 1.17-2.22), cerebrovascular disease: 2.39 (95% C.I.: 1.87-3.06), COPD: 3.94 (95% C.I.: 3.20-4.85), chronic kidney disease: 2.80 (95% C.I.: 1.91-4.11), dementia: 2.55 (95% C.I.: 1.74-3.74), hemiplegia: 1.89 (95% C.I.: 1.03-3.48), cancer (without metastatic): 2.30 (95% C.I.: 1.58-3.34). Thus, elderly patients with co-morbidities of cerebrovascular disease, COPD.
Conclusion:Since the vaccines of streptococcus pneumonia started to applied from 2005 in Taiwan for elderly patients, the incidence of Pneumococcal pneumonia was decreasing. Our results proved this policy was effective. Several researches have shown the similar evidences for the co-morbidity risk of elderly patients with Pneumococcal pneumonia, but rare studies indicated the patients with dementia was high risk group for Pneumococcal pneumonia.
