Comparative Antimicrobial Efficacy of Alcohol-based Hand Rub and Conventional Surgical Scrub in a Medical Center

Hand hygiene is a cornerstone of aseptic technique to reduce surgical site infection. However, the compliance of conventional scrub might hindered by skin damage, allergy and time consuming. Alcohol-based hand rub was shown to have satisfied antimicrobial effect, but mostly in the laboratory settings. Our aim is to compare the conventional surgical scrub against alcohol-based hand rub to evaluate the antimicrobial efficacy.
During 1 June 2010 to 31 July 2011, there are 128 health-care workers enrolled and used either alcohol-based hand rub or conventional surgical scrub as preoperative hand antisepsis during their routine practice. Hand sampling for cultures were performed before and after operations.
The culture positive rate of the alcohol-based hand rub was 6.2% before operations and 10.8% after operations, both rates were lower than the conventional surgical scrub. Most of the identified pathogens were gram(+).
Alcohol-based hand rub was more efficacious than conventional surgical scrub as surgical antisepsis and had sustained efficacy. We suggested the alcohol-based hand rub could be an alternative surgical antisepsis in the operative theaters.
