投稿標題(英): Reduction in catheter-associated urinary tract infections by bundling interventions in a neurological ward of Medical Center

投稿摘要: Catheter-associated Urinary Tract Infections (CAUTIs) are a common and costly worldwide problem, which is reflected in the neurological ward that bladder dysfunction is often managed with urinary catheterization. The purpose of this study is to develop, implement, and evaluate a catheter-associated urinary tract infection bundle. All patients admitted to the neurological ward during a 3-month period were included. We conducted a prospective before-after surveillance study of CAUTI rates on 99 patients hospitalized in a neurological ward, during 4,178 bed-days. The study was divided into the baseline period (Phase 1) and the intervention period (Phase 2). We implemented a multidimensional infection control approach that included: (1) a bundle of preventive measures, (2) education, (3) process surveillance, (4) feedback of CAUTI rates. The urinary catheter utilization rate dropped from 23.9% to 12.2% during the intervention phase (p ＜ 0.001). The rate of CAUTI was also significantly reduced from 8.7 to 0.0 infections per 1000 catheter days (p ＜ 0.001). Our study showed that a comprehensive UTI prevention bundle along with a continuous quality improvement program can significantly reduce the duration of urinary catheterization and rate of CAUTIs.
