Successful reduction of contamination of blood cultures through a multidisciplinary teamwork

Background: Contamination by skin flora during blood culture sampling leads to inappropriate antibiotic use. High contamination rate up to 6.4% was noted at Emergency Department (ED). Methods: A team including ED, laboratory, and infection control was organized in June, 2014. The workshop and checklist were set at ED from Sep 2014 to Jun 2015 for hand hygiene, bottle top sterilization with 2% chlorhexidine, sampling with Luer adaptor for 5 ml per bottle, and mandatory 2 sets of culture per sampling. Results: The contamination rate was decreased after intervention in hospital (3.44±0.88% vs. 2.28±0.65%, p<0.001) and at ED (3.77±1.29% vs. 2.19±0.92%, p<0.001), and so were compliance rates of 2 sets of cultures in hospital (24.4±5.2% vs.63.1±12.0%, p<0.001) and at ED (7.8±1.8% vs.88.7±19.8%, p<0.001). The rate of 5 cc sampling for each bottle was not changed (54.4±5.5% vs. 62.5±10.0%, p=0.306). Conclusion: Blood culture contamination may be ameliorated with multidisciplinary approaches.
